If you would like to enter then please complete this form and bring it along with you on Wednesday the 18th June. For entrants under the age of 14, the parental consent section must be completed and signed by a parent/guardian.

Please note: Forms will be available on the night but it is usually very busy so by bringing it already completed we will save time on the admin work!
